it looks like there is a column added to a table w/o checking first, whether it already exist. it should be save for you to ingnore the issue.

I would but the install process will not add the module in DNN version 6.0.1 until it installs sucessfully.

I found my problem, the survey module will not install with the DotNetNuke.Classic.Package_01.00.00_Install.zip skin that I am using, It will install with other skins applied and then do the install. the problem is that the Classic this skin has the layout I need. I am working on trying to solve this problem.
