If I have a document in the repository, is it possible to create a hyperlink in another website page that when clicked by the user causes the document to download right away, ie. without the user needing to view the details of the document in the repository and then click download in the repository? - kind of like a permanent link to download a particular document in the repository from anywhere in the site.

Is there a way to work out the actual link to the document itself?, ie. not to the item listng in the repository.

No, to my knowledge, this is not possible.

The short answer is no.  The repository relies on a postback event to download the file and it cannot be linked directly.

The long answer is potentially, depending on your desired effects.  if you want to just offer a download WITHOUT tracking you could find the actual file in your file system and link to it.  However, most individuals find this defeating the purpose of the module.
