  1. Yes
  2. Database, date is stored in a serialized format
  3. Yes and No - The UDT connector should work also with Reports, however the installation of new connectors is broken in Reports within DNN5
  4. No

As you see, you get support for form and List in this forum. The team is working on the documentation, and there will be more samples on www.formandlist.com in future.

What is the name of the database table?

All data from Form and List  or UDT is stord inside tables beginning with UserDefined*. Please note that the information is serialized, and we don't encourage anyone using these tables directly using SQL. There will be a Form And List Source for Reports in near future.

You can use SQL Management Studio to explore the DNN database.  The are several modules available (some free) that will allow you to display the contents of the any database table (DNN or Other).  The Reports module for V5 will.  Several other such as SQLView from www.dnnstuff.com or SQLGridSelectView from www.tressleworks.ca

AS for using creating UDT and then reading the table directly is not recommended -- as noted by others.   If you have access to SQL Management Studio, you can create your own tables (seperate database?) and display the data as required

Hope this helps
Paul.
