Did you enable caching? You MUST not.

Did you try http://www.codeplex.com/identityswitcher? This tool allows you to switch identity, ideal for resolving permission and caching related issues.

We have the latest version of the module installed (5.1.3) on version 5.4.4 of DNN. When we first built the forms they worked fine however we no have around 1,590 records (each with around 6 fields and a "calculating column") sadly the module is now taking 60seconds to load everytime. We have read this thread and done the bits suggested, including turning cacheing off in the Host settings. Any ideas anyone.

You should enable caching on host level. I referred to module caching In module settings or host->modules). 1590 records doesn't sound much, though it depends a lot on the data types used. You would not have those issues with 5 text fields.

I am currently trying to put together a database list for one of my clients. There is aprox 200,000 rows x 12  columns and growing.. Is UDT/ Form and List and dot net nuke able to handle something so large? I have been reading all the posts about performance and it makes me worried.

If it is only string and numbers, FnL should be able to handle 200.000 rows.
Though the module was not designed for large row numbers, as it always loads the whole table into memory. DotNetNuke itself could handle billions of records in a database, this only depends on the design of the module. I would consider using a custom module.
