I want to have an image field and an attachments field in my form and I want to be able to upload mutiple attachments and images for a specific record. Can this be done and how using the form and list module?

As far as I know, there's no such an option with Form & List. There're commercial modules which will allow you to design a form with upload capabilities (for instance, Data Springs Dyn Forms).
I wish the next F&L version includes this feature. Does anyone know when it's to be released?

you may use multiple download columns per record, each will display an URL control allowing the user to upload to any folder he has write permission been granted for, but I admit, this is not a real "uload" function. You may create your own data type for this purpuse.

You can use iframe for multiple file uploaded and didplay the list at parent page.
