What kind of field type are "Contact Info" and "Your Concern" on the feedback form shown as template.  I would like to be able to style them as in template.

I have tried to RE-CREATE the form as shown in the Gallery, since I tried to download the Feedback form per below steps and haven't been able to accomplish them.

Right now, what I'm stuck on are how to achieve the effect of the above fields.

(BTW, I tried to download the Feedback template per below steps:
    1.  Save as (when click on disk icon in Gallery beside Feedback Template):
                   TO:   \Template directory off my Portal\0
                   WITH Name of:  Feedback.Template, Feedback.Module.Template, Feedback.dnn_userdefinedtable.module.template.
                           Have tried all 3 names.
    2.  When add Form and List module to page, I do NOT see this template under any of these names.  Could someone give me clue what I'm doing wrong?  I have gone to Action menu and scanned.  F&L still doesn't see the template.)  I presume I would APPLY the template, if I were able to see it?

    3.  Just to make sure, I exported the template I am currently re-creating.  It looks like the ???.dnn_userdefinedtable.module.template file name and IS shown under the APPLY section.

Thanks for any suggestions!

Solution for Newbies:

Add "Contact Info" as SEPARATOR Field Type. 

Then in associated  'Advanced Column Options section '(below 'Data Table Setup'), checkmark the box marked:
'Acts as Fieldset (applies only to CSS-Based layout.)'

You'll now get the box around the entire area with a little box (or whatever your CSS tells it to do) around the words "Contact Info"
