The module is installing fine, but when I try to add it to a page I'm getting the error "A critical error has occurred. The type initializer for 'DotNetNuke.Modules.UserDefinedTable.Components.DataProvider' threw an exception. "  Thanks for any help you can give me.

Please check the Event Log for detailed error log and post the log here.

which module version and DNN version are you using?

Here's what I get out of the error log, the forms and list version is 6.0.2, DNN 06.01.05
