I want to create a form for students to Upload ZIP files with Description

I create 2 Columns

 - Type "Text" for description

 - Type "Download" for  upload ZIP files

I have 2 problem

1. how to config list , student see and edit just own records , and don't see other users records

2. in "Download" Column , how to config File Location , just show "My Folder" , or by default show "My Folder".

(I don't want student can see contents and files at other folders , and don't see other student records ) 

How can I fix this

 No I didnt check Repository ,

I need create a form with some fields such as , file upload (Download Type) , description (Text Type)...

yes , I set that.

but , that check box just for "manipulate " , users can see other records but cannot change other records.

that check box just limit the Editing other record , not any limit for view other record.
