I had the upgrade notification in my Extensions page, so I decided to upgrade. It would not let me. I was getting a fail on duplicate primary key:

SQL Execution resulted in following Exceptions: System.Data.SqlClient.SqlException (0x80131904): Violation of PRIMARY KEY constraint 'PK_ModuleSettings'. Cannot insert duplicate key in object 'dbo.ModuleSettings'. The duplicate key value is (1013, XML_SourceProvider).

So, I uninstalled the extension figuring I would start fresh (I could easily replace the instances I had). Upon trying to install fresh, I get:
