thee seem to be duplicate valued in your email queue. I suggest clearing the EmailQueue table first and re-performing the upgrade

No such table. I found emailqueue_tasksemail and emailqueue_tasks, but both tables are empty. Note that Forums was not installed before I loaded this module.

The problem is the following sub-query:
id = ( SELECT cdefault FROM syscolumns WHERE name = N'DateAdded' )
which will return multiple values if any other database tables include a DateAdded column.
