I'm not sure if this is related to my issue with attachments or not, but I went back and reviewed my site log afterwards and noticed the following exception.  I'm not quite sure how to reproduce it though, sorry.

What version of DNN and what version of the module.  This is not related to attachments, this is related to the breadcrumb generated for navigation.

Hey Crispy - I hit the RenderGroups exception, this time in the debugger.  Its in ForumGroup.vb, Function RenderGroups, Line 567.

If CType(ForumControl.Settings("groupid"), String) <> "" Then

ForumControl.Settings = Null

Looking at Settings, it looks like if ViewState("ForumSettings") is null, then Settings will indeed return null.  Since I'm just beginning to dive in your design, I have no suggestions at present why this happened or how to fix it (where Settings should be set).
