Thanks for your reply Mike !!!

i have did the same way as you said. but now after filling all the details in extesion settings of configuration module when i click on Update Authentication Settings then it is showing me this error.

Error Error: is currently unavailable.

Could you please help me to avoid this error. i dont know what i am doing wrong.

That error looks like you need to use impersonation in your web.config. It's covered in the documentation for the DNN 4 version of the provider and still applies to the DNN 5 version.

If i made changes in web.config file it is showing me this error.

Did you give the user you used in impersonation the same rights to your DNN install as Network Service? And also the same rights to your Temporary ASPNet (usually C:\Windows\Microsoft.NET\Framework\v2.0.50727\Temporary ASP.NET files) directory?
