In table DNN_AUTHENTICATION there is an entry for the custom provider. I also found an entry in the table DNN_PACKAGES. Am I missing something?

It doesn't sound like you are but I could be missing something too. At this point I'd suggest grabbing one of the other supplied providers and comparing it to yours to see where things might have gone sideways. I'd offer to take a look at yours for you but I"m up to my eyeballs trying to get our labs ready for students and am going to be tied up pretty much all the time for the next few weeks.

I've solved the problem: my namespace was incorrect. Thanks for helping me out.
