Looking through the 2014.01.20.log.resources file, I see a lot of these errors:

Any ideas on what may be causing this?  I've reinstalled the module multiple times.

is this a new install? What version of DNN and AD are you using ? If it's a new install I'd suggest setting the web.config to use encryption before installing.

No, this system was originally installed in 2007, with a nuke-and-pave in 2010. It has been upgraded since then.

This error occurred after upgrading from 7.1.1. to 7.1.2, with the RC version of the AD provider.

there was an issue with password history that was resolved in 7.2.0 that had a knock on effect to AD - however this is a different exception from those reports (though those provided details from the event log not log4net so perhaps it's the same issue.

Ahhhhh, so the better solution would be to jump to 7.2.0 or 7.2.1 then...
