Maybe I have this problem becouse I have installed one of those free moduls that are displaying the name of the page in the URL (to make it search-engine-friendly).

I am using DotDesk-UrlTitle from http://www.dotdesk.com/portal/HelpDeskDownloads/tabid/177/Default.aspx

try removing it and seeing whether it fixes your problem? I don't think that module should do what you're describing but without testing it out I wouldn't know for sure.

Thanks Sanjay!!! I will definitely like to help. I feel honored with the offer.

I'm currently developing 4 different websites within the same portal base so we can develop and share public and custom modules, that is how I got into the issue when adding the feedback module within a different portal. The dotnetnuke version it was originally installed with was 3.0 and has been upgraded ever since up to the latest version. I haven't tried to uninstall the module and reinstall it... but I'll try and let you know.
