Well, i'm not 100 percent sure yet, but I think it has something to do with the value for the attribute databaseOwner in

    <data defaultProvider="SqlDataProvider">
      <providers>
        <clear />
        <add name="SqlDataProvider" type="DotNetNuke.Data.SqlDataProvider, DotNetNuke.SqlDataProvider" connectionStringName="SiteSqlServer" upgradeConnectionString="" providerPath="~\Providers\DataProviders\SqlDataProvider\" objectQualifier="" databaseOwner="" />
      </providers>
    </data>

When I fill in dbo for databaseOwner, then install is not a problem. When a use an empty string, I get the error.  Somewhere  the databaseOwner attribute is not correctly used, and therefore a call to GetVersion fails. Probably all others than dbo (the default) fail.
I hope to give more details later. By the way, I'm using the 4.5.5 install package. The only things changed are the connectionstrings in the web.config, and the databaseOwner attribute.

I did a bit of the same thing; however, instead of dbo, I added the <hostname>\Admin account I added when I created the account.  I am in a hosted environment, and cannot add/change dbo.  However, when the Admin account was given the permissions as outlined in the Install doc, and then this was changed in the web.config (along with the connection string as previously posted), it FINALLY worked.
