I'll give you an example:

You've a DNN 4.5.1 a portal with an PortalID == 0 (-->  HomeDirectory == Portals/0) on a machine A. You want delete this machine but transfer this portal to on another machine B. Machine B has 5 portals. First you must add to entries on the DB on machine B: one entry in the table Portals, the PortalID should be 5 and name your HomeDirectory Portals/5 and a second entry in the table PortalAlias with the portal aliase from the machine A and the PortalID == 5. Then do you copy the directory on the machine B to the directory ./Portals/5 and check the permissions (ASP.NET user !).
Normally after a restart the migration is finished.
I'm not 100% sure but when you copy a 4.x portal to another 4.x DNN machine everything should work.
