I can confirm that I've had massive issues with "add existing module" as well.

Adding an existing module to another page made me unable to delete the original module.

Seconding the need for a core fix.

Sorry, I should have left a link to the issue.

Going through Gemini I see nearly everything is checked in and completed for 4.5.4. Can someone indicate if this issue can still be addressed in 4.5.4? This is literally a "Show Stopper" for us.

Yes, I agree that this is a serious issue that needs to be fixed. I have been able to reproduce in a clean 4.5.4 install:

1. Create new 'Test' page
2. Specify Add Existing Module in Control Panel
3. Select the 'Home' Page and select the Links module
4. I select 'Right Pane' and click 'Add'
5. The module instance is added to the Right Pane on the 'Test' page
6. However the module instance was also moved to the 'Right Pane' on the 'Home' page ( it was suppose to stay in the 'Left Pane' )

I will investigate further...

In reviewing the underlying data, there is no problem at the database level, so this would seem to point to caching as the problem.
