Mike, please note that web gardens are not a supported scenarios by DotNetNuke, please use a web farm and have a look into the appropriate document, availabe for download on Resources > Documentation > Downloads page above.

if there are multiple applications running in the same app pool, this might be a reason, did you try to separate DNN into its own? How often are app pools be recycled, are there any issues logged in Windows Event Log?

I had changed from 1 to 5 maximum threads recently, and began getting these problems.  I ahd made changes to the cache settings at the same time, and thought that was to blame for the problem but I just read this thread, changed it back down to 1, and that fixed it.  Note it does require an IIS restart (not just an app recycle).  Thanks for the useful thread!

Indeed the multiple worker processes seem to be the prob.
