Ali Cart has nothing to do with it -- Bruce Chapman mis-wrote his friendly URL provider.

I had to comment everything out of web.config, run the install, then I got an object reference error, then I put back all the friendly URL stuff and the site's upgrade shows as completed.

Also, the 4.9.1 sql provider tries to add a Name field to the Version table, and this field already exists, so these lines of code in the sql provider must be commented out for the upgrade to run properly.

The iFinity Friendly Url Provider indeed has a bug in it that doesn't support install.aspx?mode=upgrade.  This has been fixed as of version 5.00 of the iFinity Friendly Url Provider.

If your site is running anything prior to version 5.0 of the Friendly Url Provider, please disable the provider in the web.config as Tom has done, then re-enable once the upgrade is complete.  Alternatively, upgrade to the latest 5.00 version prior to attempting the DNN upgrade.

The same applies for the iFinity Url Master module : prior to version 1.13.00, it does not correctly support a DNN upgrade.  You must disable the provider by clicking on the 'revert to standard provider' button, perform the upgrade, then re-enable the provider.

Alternatively, upgrade to the 1.13.00 version of the Url Master module, and you can reliably upgrade your DNN installation in-place.
