there is no automated way. you can repackage manually or transfer the module to a dnn4 installation and use the built in packaging wizard to create a dnn4 package.

Thanks Sebastian! Can you give me more help about manually repackage? I must to work on .dnn file? Or is it possible repackage it after installed on DNN5 for DNN 4.x destination?

For example, here you can find free version of simplifed user registration with source for DNN5. How I can repackage it for DNN 4.x?

if the DNN 4 is on localhost, install the dnn5 module by copying over the files and (manually) recreating metadata in Host :: Module Definitions. if the module is working properly, enter module definition again and use "create package" link at the bottom of the page.

Please be aware, that the other module listed "form & List" uses new features of DotNetNuke 5 and will not run properly on DNN 4.
