The module they used is called "ContactUS" and it has two corresponding files associated with it: Settings.ascx and ViewContactUs.ascx. When it is selected (say, if I try to place it inside a new content pane) all personalized contents of it are immediately placed. All you have to do is add the module to a pane and then View the page and *boom* everything is there, all check boxes, and radio buttons, personalized text, etc. are in place -  just like it is currently viewed when anyone visits my client's site.

I can not see anywhere to go into edit any of it. So I figured they must have somehow built the entire module very specifically for this one use.

I can not locate anything editable from being logged in to the Host control panel but it seems like this ContactUS module file must be available somewhere for modification. The server admin *did* send me the .ascx files -  "ViewContactUS.ascx" contained all of the HTML elements of module (and I easily found where I could add the checkbox I need to add) - but we were thinking that modifying this .ascx file and then uploading it would not work. Or will it?
