As I don't know what or how extensive would be the changes to the look of the form and as my background is far less in design than programming, I can't offer much help there. The Feedback form makes use of a table-less design so should be able to be drastically modified in appearance by modifications to module.css. I have found that it is often helpful to load the form in IE or Firefox then using either the IE Defeloper Tools or Firebug respectively look at an play around with the css styles for each element.

The newest version of the Feedback module is excellent - In particular I like the ability to have several instances of the module with each having it's own Scope. We can now use the module on different pages of our web site and have each instance deal with a particular kind of feedback - one for requesting a particular service, another for a general "contact" request, etc. There is one minor change I would like to see - for the Subject line, an option to have a preselected Subject that the user cannot change exists, but when this is selected the Subject line must be hidden. I'd like to have an option to display it without allowing changes. I've downloaded the source and will take a look at doing this but any suggestions would be appreciated. Thanks, William

Is it possible yet to add a custom field? I'd like to just add a field for Company Name.
