What does 'Go to first unread post ' link do exactly? Does it go to the new reply in a previously read post?

This will navigate you to the latest unread post directly. There are several considerations when doing this, the user's post page size, number of posts, exact ID of the post as well as an anchor to scroll you down directly to the post (based on your user reads). If there are no unread posts for a particular user, they won't see the link.

Do you mean a reply to the selected post or a new post? If the latter, what's the difference between clicking the link versus the post title? Because both will open the selected post. "Go to first unread post' seems never goes to another post than the selected post.
