My guess is that a SQL Script did not execute that was needed for the upgrade. Was the error during or after the upgrade?

Normally at the start of an upgrade the assembly version would be higher than the database version but your post shows:

Upgrading DotNetNuke
Current Assembly Version: 04.09.03
Current Database Version: 05.03.01

Were you trying to downgrade back to 04.09.03 or was there a possibility that the dotnetnuke.dll assembly file from the 05.03.01 package did not get un-zipped and placed into the site's \bin folder when you copied the files of the upgrade package over those of the existing site. Downgrading is not supported.

Do you have a backup of the site's file system and a backup of the database so that you can restore the site back to its last working version?

In DNN 5.3.1 in "GetPortals" procedure in database, a new perameter has been added but not used. that is why when ever you are using "GetPortals" procedure from code you have to add once additional perameter, may be a constant value as it is not getting used in SP. In 5.4.1 they have removed this useless perameter.
