Hi all. I was wondering whether it was possible to specify that certain fields in a form should become required and/or visible depending upon selection in a radio button list or drop-down list or a true/false check box. For example, only showing alternate address fields if the user selects that the address they want an item delivered to is different to their home address.

No, this is not possible. 
Btw, there is a work item on codeplex that matches your request. Currenly I have no idea how to implement this feature. Any one here who like to volunteer?
