Chris, I wound up fixing the issue by comparing data to a fresh install. During my comparison, I found that the IsDeleted flag on the TabModules table was set to 1. I'm not sure how this would have happened and I didn't know how to set it back through the UI, so I updated the flag in the database and my page was back. Can you tell me how this can be turned on or off in the UI? Is there anything else that needs to be updated along with this flag? Thank you very much for your response. Jeff Banschbach

From the Recycle Bin under the admin tab you can "Restore" a page, that should turn off the isdeleted flag for any pages you restore.
