EVERY SINGLE HELP IS HIGHLY APPRECIATED - PLEASE HELP !!

(Administrator - my first approved thread is not visible on the forum as forum is throwing error - violated foriegn key for my thread and hence reposting this one with a different title)

Gentlemen- this is the first time I am every trying to install DotNetNuke 4.0.3. Infact this is the first time in my life that I am working on this framework and I am struggling little too much on this one. Its been over 2 and half days but no results and I am kind of running out of time to hit my deadlines on configuring this framework. Every single guidance and help is very highly appreciated.

Following is the detail description/configuration of my installation and the problem that I am facing:

OS: Win XP Pro SP2
IIS: IIS5.1
Database: Microsoft SQL Server Express Edition with Advanced Services
Database Version: 9.00.2047.00

Following is completely hypothetical equivalent of my configuration. Administrators please do not delete or remove this  - none of this is the actual true configurations but just equivalents to explain my situation and relate it to web.config and help me resolve my issue.

Machine Name: MCH
Machine Admin Username: MCHAdmin
SQL Server Name:
MCH - note this and machine name is same - we had installed SQL with default settings !!
Database Name:
DotNetNuke
Database Users:
MCH\ASPNET - db_owner
                         NT AUTHORITY\NETWORK SERVICE - db_owner
Database Default Owner:
dbo - this is username in db for user MCH\MCHAdmin - my os admin login
Database File Name: DotNetNuke.mdf
Database File Location:
C:\Program Files\Microsoft SQL Server 2005\MSSQL.1\MSSQL\Data
MS SQL Server: Enabled for local and remote connections over TCP/IP and Named Pipes protocols
MS SQL Sever Authentication Mode: Both SQL Server and Integrated Windows Authentication (which we want to implement for setting up DNN 4)

DotNetNuke IIS Installation Directory: C:\InetPub\wwwroot\DotNetNuke_2
                                                  -- both MCH\ASPNET and NT AUTHORITY\NETWORK SERVICE have full permissions over this installation tree throughout.

Following is my web.config:

I have tried every single possible combination to resolve this but no luck and now I am hitting my deadline for this assignment  - gurus need our magic out here !! WHAT AM I DOING WRONG??
