When it comes to deciding on the best option I think you need to look at your project and then decide if there's an option for a custom design? If so then you're starting from scratch & when it comes to skinning it can be focused towards DNN5. If not then I'd be inclined to look for a DNN5 template skin that uses the best practices of valid XHTML, using CSS/DIV based code with no tables and a list based navigation, ontop of this if the containers are semantic (i.e. they use H1, H2, H3 tags for the titles) that's also a bonus for SEO and usability.

As mentioned DNN4 skins will work with DNN5 so depending on the requirements of the project and the goals of the site, budget e.t.c. may determine the route you go down. One thing I'd mention though is that designers and developers are always maturing their skills and techniques, so something built for DNN5 has a good chance of being better quality and using more up-to-date techniques than one built for older versions. This said have a look at the reviews for skins because the best critics are the people implementing them.
