is it sql server or web server which causes cpu utilization?
if sql server: please monitor which queries are executed.

The CPU for the SQL is normal, the whole issue is with the w3wp.exe IIS worker process, when it reaches 100% on all 4 cores it gets stuck. When I kill the process (w3wp.exe), and in less than a second, it restarts without an issue.

It is very weird

this sounds like a caching issue or a data structure anomaly. unfortunately, w/o a real error this is hard to identify.

I had first static and dynamic compression and out caching for .js and .css enabled.  But All disabled from the IIS and site level now but still the problem persist. I cleared the cache as well

It is more likely a data structure after content translation.

I will be deploying the same portal, (same files and same DB) on an offline server with the same specs and test it.

To determine if it is an IIS or web portal/DB issue, although suspect the latter.

Will post the results here, after testing!
