Can DotNetNuke be set up where we have one database server and multiple front-end web servers?

There are two possible reasons we would want to do this.  One is to use the front-end web servers in a cluster for reliability and scalability.

If so, are there any special configuration issues?  I didn't see anything like this in the manuals.

This is definitely something that can be done, typically you will want to do this is you absolutely need a load-balanced environment. You can still have redundancy without the need to have multiple web servers. We actually have a large number of clients who are currently in this type of similar configuration, although we have some proprietary software that streamlines the process of moving data between web servers and database servers.

I would love to help you out in any way Tom. If you have some time we can discuss this at your convenience, drop me a line anytime here is my email address: My Email

Hope this helps!
