Mike have you had a chance to look at the web.config to see why the authentication won't persist across browser sessions?

The only thing that's standing out for me is that in the <httpModules> section of your web.config the second <add name="Authentication.... /> is commented out. This will prevent the provider from automatically logging users in when running in the classic integration mode. However, that said, I'm not sure why you're getting redirected to the login page when you're first going to the site. It should just send you to your homepage.

Uncommenting that line takes the user to: https://www.mydomain.com/DesktopModules/AuthenticationServices/ActiveDirectory/WindowsSignin.aspx

and throws a 500.19 error (screenshot sent via email)

Woohoo! Got it working!

I had to change my %windir%/system32/inetsrv/config/applicationHost.config file to allow overrideModeDefault for

section name="anonymousAuthentication" overrideModeDefault="Allow"
section name="windowsAuthentication" overrideModeDefault="Allow"

Both were previously set to "Deny".
