 I have noticed that the Windows Identity Framework (WIF) can makes issues with DNN, also after uninstall it.  So changing to medium trust can solve any IIS issues.

 About I done several Upgrades to DNN 7.0 and 7.1 w/o issues, I am sure the Problem is to search in the configuration of IIS, .NET and permissions!  Perhaps sometimes also some issues comes up if the prerequest are observed.  So I would check prerequest, permissions, .NET and IIS configuration.

please make sure you are running you application pool on ASP.Net 4.0 integrated.
Also clear local browser cache and cookies.

I have tried changing to Medium Trust and that doesnt fix the problem
The app pool is already running v4 Integrated
Any other suggestions?

please make sure you log it, I don't recall seeing this as a reported error yet - though its probable we would need a copy of a site where it is failing to debug it as looking into ActionButtonList and the ModuleActions property the only way that could be triggered is if there were no associated actions with the module -which suggests a data problem. As we haven't seen others like this is could well be related to a particular 3rd party extension .
