I was able to get things working by deleting the folder, the database, and the IIS entry and recreating them from scratch. I didn't do anything differently this time around, so I'm not sure why it worked.

Exactly what I've had to do in the past and why I suggested it. I'd love to know what causes it but figured it wasn't worth the time and just deleted and reinstalled everything.
