We recently upgraded our company website to 07.03.02 and have been testing everything on a Windows 7 system.  I have moved it to a fresh Server 2008 x64 web edition and was at first getting the home page but no sub pages.  After some googling I installed a Microsoft patch for extensionless URLs and now the site runs perfectly when browsing locally using localhost.  If I try to browse if from another workstation I get 404's (IIS 404's, not DNN).  The home page will not even load.  I turned on Failed Request Logging and it appears the URLRewrite module is causing the problem.

Any ideas?

I just nuked the Server 2008 and am in the process of rebuilding using Server 2012 R2.

I am now running IIS on a Server 2012 R2 server and am getting the same errors.  I checked and anonymous authentication is enabled.  Here is the section of the trace that shows the error:
