In the Windows Event Log I had the following:

Exception message: Violation of UNIQUE KEY constraint 'IX_Version'. Cannot insert duplicate key in object 'dbo.Version'.

First attempt to login displays a message that a Critical Error has occurred and to check the eventlog.... which I can't access because I can't login. Second attempt loads a 500 error in the pop-up login box.

duplicate key in version table might be caused by your upgrade attempts, i'd simply ignore it.
I always remove unused modules and upgrade installed ones (if available), prior to a DNN upgrade.

If you can get at the database you can look at the eventlog table directly. You need to sort by CreatedDate (or some such). You probably need to look at several rows, and the XML does you no favors.

Finally got the portal upgrade to work.... Now I can't access my module settings. Get "Object reference not set to an instance of an object." error. Event Log shows:
