I am having the same issue with DNN 7.4.2

I see the users' first and last names in the Users table, but when I load the user into Manage Profile, the first and last names do not load. Many users are requested to add their first and last names again.

Any update on this issue?

starting with DNN 7.4, leading values for firstname and lastname are stored in Userprofile table instead of users table.
you might need to copy them over from users to userprofile table.

1. The new users are not consistently added using the UserController.CreateUser(). Some have the first name, last name added to the users table and some do not. Again there is no rhyme or reason that I can find why this is happening.

2. The bigger problem from this issue comes when deleting a user account. The emails are sent to the admin's that the user has been deleted but because there is no first and last name then the admin doesn't know who was deleted by the email.

Here is the DNN generated email that I get when I delete a user that didn't have the proper name values entered.

please check the template for the mail in Admin > Languages, using the pen for static files in site or host level.
The language editor should display globalresources as default file, and there should be an Email... key with the template above. check the placeholders being used and compare with default template displayed in the right hand column.

It appears to me that this is a DNN translation error. If you are saying that the user profile table stores the first name, last name, etc. Then where does DNN translate those tokens, because the format for this email is correct and the tokens match.
