Bump on this issue. I checked on the issues page and this item was on the top of the list and listed and major, but not yet assinged and no feed back on it.

Turn it to premium... so your portal admins can't see it and use it and thus freak out and b@#$% about it to you.

Login as host...
Go to Host -> Module Definitions
Find the Feedback module. Click the pencil to edit it.
Check the box marked "premium."
Make sure none of your portals have it in the box that says "Assigned."
Click Update.

For the record - if you're just going for a "contact us" type form, I like SixxacDnnFeedback - its free.

Yes DNN developers - please make this a priority fix!   The module is essentially unusable until it's fixed. 

Even when adding multiple feedback modules in the *same* portal, the feedback messages duplicate (or rather, as the other person said, there's no unique identifier on the feedback record to distinguish it from another instance).

Other than this critical bug, it looks like a pretty nice module!  ;)

If anyone needs a more fully-functioned feedback form (closer to a guestbook, i guess?) then you can go subscribe to http://www.ventrian.com - those guys have one that looks pretty cool - and its like $20 for a three-month subscription to their site... I presume that means you can download the module and use it as long as you like, but you only get the updates for three months... I would certainly hope thats the case...

Anyway - it looks MUCH closer to what THIS module SHOULD be, but isn't...
