 Many thanks for the quick response.  Deleted the two DLL mentioned and are back up and working

To answer you questions,

• The survey module had never been used before - this was a first time upload & install
• The version of DNN is 4.03.04
• The version of Survey was 04.00.50
• The versions of the DLL's deleted were, Survey 3.3.0.27701 and SqlDataProvider 3.3.2323.27701, both were dated 23/6/2006
• .NET version 2.0.50727.42

Thank you that is very helpful. That version of DNN is unable to properly process the .txt file that tells it to remove the assemblies from the previous version of the Survey module (The Survey module was automatically installed in that version. This is no longer the case).

DNN 4.03 is outdated and there have been some security updates. I would advise you to upgrade.

Thanks for the help. Will follow your advise and upgrade DNN (to 4.5) first , then install the latest Survey module
