FYI (to everyone else)...  Through some upgrade testing I have been doing this week, I have found that depending on how you back-up your original version of the web site, you might need to reapply the permissions.  Now, you might be able to check the permissions on the root folder of the web site, but you then need to make sure that you click the "Advanced" button and do two things:

1. Ensure that the root folder is not inheriting the parent permissions (choose "Copy" if you need to uncheck this box)

2. Check the next check box to replace the permissions on the child folders and enable propagation from the parent.

This has fixed some previous issues when I was testing.  It will ensure that all subfolders has the Network Service or ASPNET account has the appropriate permissions on all of the subfolders (provided that the parent or root folder was properly configured to begin with).

This is only happening on my installations when a file has the same name. Can we get an "Overwrite Existing Files" option? This is a major pain for many of my users.

Argh!!!  I am now running into this now.  This needs to be updated as soon as possible.  Does the Documents Module Team have an ETA on an update?  I doubt it.  This appears to be a problem with the secure file system, and maybe not the module itself.  :(

This new secure file system is the most frustrating thing right now.  We need a smillee face  in FTB that looks exhausted...

I put it in Gemini today. I can't believe that nothing has happened with this issue. I have the problem since my 3.2.2 install. This makes the file manager worthless, IMO.

I don't know about worthless, but this whole situation certainly hasn't made my life any easier.  :)
