David I have do look on the 2nd Link you posted on the beginning of this thread.  There is a "Your IP" Module.  Please try do uninstall it from the Page.  I think this Module caused your issue.

I have removed the 3rd party module (Your IP) and the problem still exists.  I know which thread has caused the problem and think it has been caused by one of my users attempting to add reply or edit reply.   The problem has never happened before and the user is one of the most frequent users so I don't understand why it has happened now?

So your problem is resolved, I will mak this thread as resolved.  I can not reproduce it.

No its not resolved.

I have just narrowed the problem down.  I still have no fix.

I have restored my database backup to fix the problem (thankfully backup was recent), but the question still remains.  Why does this happen and how is it fixed as it appears there are a number of instances.
