Using:  DNN version 4.4.1

I am getting this error: 

  A critical error has occurred.
Value cannot be null. Parameter name: passwordAnswer

This is happening b/c I turned on "requireQuestionAnswer" feature in my web.config file.  I created a new user, entered the password question and answer on the registration page.  I am getting this error if as an admin I go to change his password or if the user himself tries to change the password.  The password question and answer fields are not visible in both cases.

Please help.
