 I just migrated from 4.9.4 to 5.1.2 and ot my surprise my DNN instance is missing admin>extenstion and errors on host>moduledefinitions. needless to say I can not install modules.

the error is as follows. I would really appreciate it if anyone could lend a hand.

During the upgrade were any errors reported in the SqlDataProvider scripts? If so, please check the log files mentioned in the error. These log files are located in the site's "Providers/DataProviders/SqlDataProvider" folder and will be named 05.00.00.log, 05.00.01.log, etc. If you find a .log file that is not empty, please report your findings here. I am particularly interested in any errors related to a failure to drop the foreign key constraint FK_xxx_PackageAssemblies_PackageAssemblies (where xxx represents the objectQulifier that was specified in your web.config file) then triggering a mass of errors involving the Packages table.

It appears that the final part of the upgrade process did not successfully complete. I hope that you made a complete backup of the site's files AND the database before the upgrade was attempted.

You may also find that there are other missing or non-functioning items on the admin and host menus.
