How do I put a login page in the header on all pages of my site?

do you want to display the login link only (this needs to be done in the skin by adding the [login] token) or the login control with username and password - which requires to add the login module to all pages.

Yes, I want the username and password fields in the header, which is not controled by the containers.  Is there something that I can use example [login] just like the search and nav?

the easiest way is to provide a login pane in your skin and place the Account login module inside, with the setting "show on all pages".
