How to create a document with additonal fields? I want the additional fields can be a text-box, list-box or a radio button?

Do I need additional Module or already supported by DNN to meet my requirement?

I suggest to use UserDefinedTable module, already packaged with DNN.

UserDefineTable is great module, thanks for help.

Also, how to:

- create dependable list box? (like Country list box related to State list box).

- a list box that get value form another table? Suppose I have table contains list of Computer Brands.

sorry, both features are not currently contained in UDT, but you can use a comma separated list as a source of any column. If you really need to represent hierarchical data, you might need to extend the module or look for commercial alternatives (e.g. my corp is currently creating a Custom Forms and Lists module where this can be done easily via GUI).
