Trying to upload in File Manger and receive this error:
"The underlying system threw an exception. The file has not been..."

When trying to use the edit feature and upload an image we have a serious issue.  We click on the "Image Manager" and the new popup window appears.  However there is nothing in the panes.  The icons such as "Back", "Forward", "Refresh", "New Folder", "Delete" and all of the others do appear but none of them work.  This is something that just started.

please go to File Manager in Admin menu, perform a recursive sync on the root folder and retry.

When going into File Manager I right clicked on "Root" and can choose:
Create Folder
Refresh Folder
View Folder Properties

If I just click on it and choose the Refresh options I can choose:
Refresh
Sync This Folder
Sync This Folder & Subfolders

Just a quick note.  I am not able to delete, rename or create anything in file manger.  This is only happening to this one dedicated DNN install. Other DNN installations on the server are fine.  This only started happening the other day.

please check file system permissions for the ASP.net user, as specified in advanced settings of your IIS App pool.
Afterwards hit Sync This Folder & Subfolders while being in the root of your site in Admin > File Manager
