When I find this on a customer site I add the module/skin/container twice to make it work. I am not sure why it happens and it's always on a remote server, so caching might be the issue.

I think the problem is in Visual Studio.  We are using VS 2005 on a 512mb laptop also running SQL server 2000. If I attempt to upload skins from the visual studio environment, it returns directly to the panel after doing nothing, and sometimes I get a URLrewrite error with an instance null.  If I start the site from internet explorer (without visual studio), it works just fine. I did make the changes to httpruntime that others have suggested.

I just wanted to add that I am having the same problem, and don't use Visual Studio.  Just using the install files.

Making the changes to the web.config fixed the issue for now, but I've only uploaded 2 files.

I noticed something in this thread that caught my attention.

Some skins are created in ASCX and some in HTM method.

Between DNN 2 & 3 there were some code breaking changes, and it required you to modify the first line of code in the code ascx file.Try copying and pasting those lines from a DNN3 ascx file and replacing the DNN2 one.

This is only relevant for the ascx method of skinning since, HTML would not have the code in there.  I've got an article on my site that explains how to convert DNN2 skins to DNN3, but that's bascially what I'm explaining just here.

So give that a try.. When working in ASCX mode it's not forgiving..

Just a thought - I might looking in the wrong diection too you know!.
