Is there any way to run some code once when a module is created on a page? I need to add a row to my database each time an instance of my module is added to a page.

Every time a module is added to a page, it executes an insert stored procedure (I forget which one). You could create a sql trigger on the insert procedure, and if the ModuleDefinitionId  = the moduledefid you want to track, execute an insert into your other table.

This would mean modifying a core stored procedure, so you would have to be aware that it may not work after a DNN version upgrade.

Another way which does not involve a core modification is to check (most likely in the module's Page_Load handler)  if the module Settings hashtable is empty or a particular ModuleSettings key returns a null or empty string. If so, since ModuleSettings are both key AND ModuleID dependent, you could then run your custom code and call ModuleController.UpdateModuleSetting to set the value of this key. Something like this:

If Settings.Count = 0 OrElse String.IsNullOrEmpty(Settings("Initialized")) Then

     'run custom code here

     Dim mc As New DotNetNuke.Entities.Modules.ModuleController
     mc.UpdateModuleSetting(Me.ModuleId, "Initialized", "True")
End If

Bill's method is the best way to go.  YOu do NOT want to try to tap into the core functions are you can risk causing major harm when trying to upgrade.
