Can anyone provide any recommendations for custom forms for DNN.

My requirements are simple, I require a form to be able to insert/update/delete data from some custom tables in my database. All data should be stored in database nowhere else.

Enterprise Forms will write to a table that it creates for you based on the form (it also creates a view as well.) It's admin interface requires Internet Exploder, but the 'public' form plays well in all browsers. Its downside is the learning curve, upside is nice workflow.

H20, by Onyaktech will also write to a table (of your choosing), however, I have found that the interface (admin and user) leaves much to be desired, and does not play well with Firefox.

I have found that writting my own is  the best way to go. If you need a good starting point, then XMod is a nice module.

You can try module from http://components.init.si. Module can insert/update/delete, bulk insert/update (export/import option using excel or access or html), create/alter tables and columns, etc on existing or new tables. You can download fully functional demo.
