I've been searching for one for my own church's DNN site but have not found any that would handle the head of family/family member relationships that a church really needs for its directory.  The DataSprings dynamic registration and dynamic membership list modules (available on SnowCovered) appeared to provide many of the other needs including photo upload.

I have just finished building a custom on-line/e-mail prayer request submission and tracking module and have it in beta testing on my own church's site.  My next project will be to build a comprehensive membership module taylored for churches that will handle both photos and family relationships plus import data from the Servant Keeper Membership Pro system that we currently use.  It's completion will, however, be many months away.

Have you seen the directory in the Smart-Thinker UserProfile module? Here is an example profile and the directory (all views are completely customisable with tokens so you can chose what to display and how to display it (including any custom core fields you add).
